The case managers are registered nurses and liscensed social workers that identify the needs of elderly an/or disabled through a comprehensive assessment completed in the homes of potential clients. The assessment is followed by the development of a care plan, then approval of the care plan by the Division of Medicaid. The case managers arrange for the approved services and provides ongoing monitoring and adjustment to ensure proper care.
